Exo-Cica/HA Complex Promotes Skin Rejuvenation and Barrier Repair via Extracellular Matrix Reconstruction and Anti-Inflammatory Regulation

Background: Effective skin rejuvenation requires coordinated regulation of inflammation, restoration of epidermal barrier function, and reconstruction of the extracellular matrix (ECM) and basement membrane. Hyaluronic acid (HA) is widely used as a wound covering material that provides a hydrated microenvironment; however, its ability to actively regulate inflammation and structural remodeling is limited. Recently, plant derived exosome-like vesicles have emerged as potential bioactive regulators of skin rejuvenation. Objective: This study investigated whether a combined Exo-Cica/HA complex (Celexo), developed as a wound-covering material, enhances skin rejuvenation through anti inflammatory regulation, structural improvement of ECM, and restoration of epidermal barrier and basement membrane integrity. Methods: The biological effects of Exo-Cica/HA were evaluated using in vitro cellular models including keratinocytes, fibroblasts, macrophages, and melanocytes. Anti inflammatory activity, barrier-related gene expression, and basement membrane associated markers were analyzed using quantitative real-time polymerase chain reaction. Wound-healing potential was assessed using scratch assays. Ex-vivo human skin tissue models were employed to evaluate ECM remodeling, basement membrane restoration, barrier recovery, and re-epithelialization following ultraviolet B (UVB) irradiation or barrier disruption. Histological and immunofluorescence analyses were performed to examine structural tissue changes. Results: Exo-Cica/HA exhibited no cytotoxicity and significantly suppressed pro inflammatory cytokine expression. The treatment enhanced the expression of barrier related markers such as filaggrin and hyaluronan synthase-2 and upregulated basement membrane components including laminin-332, nidogen, and type IV collagen. In UVB-damaged human skin tissue, Exo-Cica/HA restored collagen and elastic fibers and improved dermal–epidermal junction morphology. Furthermore, it accelerated keratinocyte migration and re-epithelialization in both in vitro and ex vivo wound models. Conclusion: Exo-Cica/HA enhances coordinated skin rejuvenation by modulating inflammation, reinforcing epidermal barrier function, improving basement membrane integrity, and reconstructing the ECM, supporting its potential as a biologically active wound-covering material.
